February 08, 2018 – The Professional Regulation Commission Davao
Regional Office will conduct a Mobile Service on February 21 to 23, 2018
at Overland Terminal, Alim Street, Kidapawan City, North Cotabato.
The following services will be offered:
- Application for licensure examinations with Online Application Form generated and printed at www.prc.gov;
- Renewal of Professional ID card with Online ID Renewal Form generated and printed at www.prc.gov.ph;
- Authentication of Report of Rating/Passing/Professional Identification Card/ Certificate of Registration
- Certifications (Certified True Copy) of Report of Rating/Passing/Professional Identification Card/Certificate of Registration.
Requirements: Original and photocopy of document to be
authenticated, Documentary Stamp and Certification fee of P75.00 per
copy.
